Position Overview
A Community Health Nurse works to promote the overall wellness of individuals and communities through education, advocacy and delivery of care. The CHN promotes healthy lifestyle choices by instituting health screenings, wellness projects and provides education on disease management and prevention.
Developing relationships helps the CHN address health disparities within marginalized communities by focusing on the environmental, physical, and social determinants of health. The CHN also builds individual and community capacity through cultural mediation and by increasing the knowledge and self-sufficiency through a range of activities such as outreach, community education, and advocacy.
Location and Hours of Position
This position will be working in Schuylkill and Northumberland County. Travel required. General hours for the position are 8:30am-5:00pm (with some evenings and weekend hours).
Compensation
Minimum start is $27.00 per hour. PHN recognizes experience, education, and site location when determining wage.
